sounds like you need to reset your database back to factory defaults.

Hold the SHIFT key down while launching PowerSDR.
You will see a message asking to reset your database back to factory default.

See if it works OK after a factory reset.

Take a photo of the full PowerSDR console when this happens, so I can see everything.

Make sure the VAC1 "mix VAC1 and Mic audio" is not ON
Otherwise you would need to look at the program you use for the virtual audio cable.
I use VB-Cable and Voicemeeter Banana for VAC

I have never encountered that fault before. if installing .263 doesnt work you may need to uninstall everything and re-install the Full installer first (again) before the .263 incremental. Assuming you already have some version of v2.8 installed and running, I would download and try to install .263
